The tool provides a comprehensive and up-to-date directory of thousands of health and human services agencies and resources available to Michiganders. It streamlines the referral process by allowing users to document resources provided and, where available, send electronic referrals directly to community-based organizations. It also facilitates follow-up by enabling users to review referral outcomes and service completion status during subsequent patient visits.

The application launches seamlessly within the Epic® ecosystem. It utilizes the patient’s address from Epic® to automatically filter search results, displaying only relevant resources that serve individuals in their local area. This makes the app easier for healthcare providers to use.
